Insurance and Billing

We highly recommend that you verify your insurance benefits before your first appointment using our benefits verification form to receive the most accurate information.  Patients who have not verified their coverage before the appointment should be prepared to pay at the time of service.  We send bills electronically to some insurance as a courtesy to our patients (see below).   However, verifying benefits, obtaining referrals,  prior authorizations, and resolving any payment issues is the patient's responsibility.  As a small private practice we unfortunately do not have the resources to communicate with insurance companies outside of submitting claims.  A copy of our fee schedule can be found here.
